PeKa B40: Free Health Screening & Medical Aid for Malaysians

What Is PeKa B40?

PeKa B40 — short for Skim Peduli Kesihatan untuk Kumpulan B40 — is a free government health screening and medical assistance programme managed by the Ministry of Health Malaysia (KKM) through ProtectHealth Corporation Sdn Bhd, a non-profit subsidiary of MOH.

Launched in April 2019, PeKa B40 focuses on early detection and management of non-communicable diseases (NCDs) — conditions like diabetes, hypertension, and high cholesterol that often go undetected until they become serious. The programme also helps cover the cost of medical devices and cancer treatment for those who qualify.

As of 2026, over 5,275 registered clinics across Malaysia participate in the programme. Despite this, approximately 90% of eligible Malaysians have never used their health screening benefit — meaning millions are leaving free healthcare on the table.

Am I Eligible?

Eligibility is automatic — there is no separate application. The criteria are:

About the STR income threshold: STR targets households with monthly income of RM2,500 or below. If both spouses are STR recipients, both are independently eligible for PeKa B40. If only one spouse receives STR, the other spouse is also covered — as long as they are aged 40 or above.

Not showing as eligible? If your household income qualifies but you are not in the system, you may not be registered for STR. Apply for STR at bantuantunai.gov.my first. Once approved, PeKa B40 eligibility follows automatically.

The 4 Benefits

PeKa B40 has four distinct benefits. Benefit 1 (health screening) must be completed before you can access Benefits 2, 3, or 4.

Free Health Screening (Saringan Kesihatan)
Free
A comprehensive health screening at any registered PeKa B40 clinic. Includes physical examination and laboratory tests:
  • Full blood count (kiraan darah lengkap)
  • HbA1c — blood sugar control over 3 months (kawalan gula darah)
  • Lipid panel — cholesterol levels (paras kolesterol)
  • Kidney function tests (fungsi buah pinggang)
  • Urine test (ujian air kencing)

Screens for: diabetes (kencing manis), hypertension (darah tinggi), dyslipidaemia (kolesterol tinggi). No fasting required. No cost to you — ProtectHealth pays the clinic directly.

Two visits First visit: history-taking, physical exam, specimen collection. Second visit: review results with doctor. Both visits must be at the same clinic.

Medical Device Assistance (Bantuan Alat Perubatan)
Up to RM20,000
Covers the cost of medical devices not subsidised by MOH hospitals. Examples:
  • Heart stents (stenting jantung) and pacemakers (alat pacu jantung)
  • Hearing aids (alat bantu dengar)
  • Artificial joints (sendi buatan) and spinal implants
  • Intraocular lenses (kanta mata)
  • Prosthetic limbs (anggota palsu)
  • Wheelchairs (kerusi roda)
  • Oxygen concentrators and breathing therapy equipment

Important: Only applies when treated at a government MOH hospital, not private hospitals. The application is made by your MOH hospital specialist on your behalf — you do not need to apply yourself.

Cancer Treatment Incentive (Insentif Tamat Rawatan Kanser)
Up to RM1,000
A cash incentive to support cancer patients who complete their full course of treatment. Paid in two instalments:
  • RM300 upon starting cancer treatment
  • RM700 upon completing cancer treatment

Paid directly to your registered bank account. Purpose: reduce dropout rates among B40 cancer patients who cannot afford to complete treatment. Application is handled by the treating MOH hospital.

Transport Incentive (Insentif Pengangkutan)
RM500 – RM1,000/year
Reimburses travel costs to and from MOH hospitals for Benefits 2 and 3 treatment only. Rate is distance-based, capped per year:
  • RM500/year for Peninsular Malaysia
  • RM1,000/year for Sabah, Sarawak, and Labuan

Credited into your bank account per approved journey. Managed by the treating hospital through the BMS (Benefit Management System) portal.

How to Get Your Free Health Screening

This is the most actionable step — and the gateway to all other PeKa B40 benefits. Here is exactly what to do:

  1. Check your eligibility online Go to pekab40.com.my and enter your IC number to confirm you are in the system.
  2. Find a registered PeKa B40 clinic near you Use the clinic directory at bms.pekab40.com.my/provider to find registered clinics. Look for clinics displaying the PeKa B40 sticker or emblem on the door. Over 5,275 clinics are registered nationwide.
  3. Walk in with only your MyKad No appointment necessary at most clinics. Your IC number is all you need — the clinic will verify eligibility through the system.
  4. First visit: history, physical exam, specimen collection The doctor will take your medical history, conduct a physical examination, and collect blood and urine specimens for laboratory testing. No fasting required. The clinic handles lab logistics and billing — you pay nothing.
  5. Wait for your second appointment The clinic will contact you (usually within 1–2 weeks) once lab results are ready. You will return to the same clinic for the results review. Do not go to a different clinic — both visits must be at the same location.
  6. Second visit: review results with the doctor The doctor will explain your results and advise on any follow-up. If a condition is detected, you will be referred to a government Klinik Kesihatan for ongoing management (treatment costs are subsidised but not free).
Both visits must be at the same clinic. If you attend a different clinic for your follow-up, the process resets and your screening will not be recorded as complete. This also blocks access to Benefits 2, 3, and 4.

Use it even if you feel healthy

PeKa B40 specifically targets conditions like diabetes and hypertension that are often asymptomatic in early stages. Many Malaysians discover conditions they did not know they had during this screening. Early detection = lower long-term cost and better outcomes.

Tell your parents about this

Uptake is lowest among those aged 60+. If your parents or in-laws receive STR and are aged 40 and above, they almost certainly qualify. Walk them through the eligibility check at pekab40.com.my or bring them to a registered clinic.

Benefit 1 is your gateway

You cannot access the RM20,000 medical device aid (Benefit 2), the RM1,000 cancer incentive (Benefit 3), or the transport incentive (Benefit 4) without first completing the health screening. Do not skip Benefit 1.

After screening, your data helps future access

Once your screening is recorded in the BMS system, your MOH hospital specialists can see your eligibility and proactively apply for Benefits 2–4 on your behalf if relevant conditions arise during hospitalisation.

Frequently Asked Questions

Do I need to fast before the blood test?
No. Unlike many private health screenings, PeKa B40 does not require fasting. You can eat and drink normally before your visit.
Can I use a private hospital for Benefits 2, 3, and 4?
No. Benefits 2 (medical devices), 3 (cancer incentive), and 4 (transport) only apply when you are treated at a government MOH hospital. Private hospital treatment is not covered under PeKa B40.
I checked my eligibility and I am not in the system. What do I do?
PeKa B40 eligibility is linked to the STR (Sumbangan Tunai Rahmah) database. If your income qualifies but you are not registered, apply for STR first at bantuantunai.gov.my. Once approved for STR, PeKa B40 eligibility is automatically granted (for those aged 40+).
How often can I do the free screening?
The programme allows eligible recipients to be screened regularly, though the frequency may be determined by the clinic and your medical history. Contact your registered clinic or ProtectHealth at 1-300-888-938 for current guidelines on repeat screenings.
Will the clinic bill me for anything?
No. All PeKa B40 screening costs are settled directly between ProtectHealth and the clinic. ProtectHealth pays the GP RM40 for your first visit and RM20 for the follow-up. You pay nothing. If a clinic tries to charge you for a PeKa B40 screening, it is incorrect — contact ProtectHealth.
Does PeKa B40 cover treatment costs after screening?
No. PeKa B40 covers the screening itself (Benefit 1) and specific medical devices and incentives (Benefits 2–4). If you are found to have a condition after screening, you will be referred to a government Klinik Kesihatan for treatment — that treatment is subsidised but not free. PeKa B40 is a screening programme, not a treatment programme.
My spouse is not an STR recipient but I am. Are they covered?
Yes, if your spouse is aged 40 and above, they are also eligible for PeKa B40 as your registered spouse. The age requirement of 40+ applies to the spouse as well.
