Step-by-Step: Replacing Your Lost MyKad

  1. File a police report at any balai polis
    Walk in to the nearest police station and report the loss. Tell the officer you lost your MyKad (bukan dicuri, kecuali jika ia dicuri — use "kecuri" if stolen, "hilang" if lost). The officer will take a brief statement and print the report. This takes about 20–30 minutes. Keep the original report — you will need it at JPN.
  2. Go to the nearest JPN office or UTC
    Bring your police report plus any secondary identity document — your passport, birth certificate, or a family member's MyKad showing your relationship. JPN counters at Urban Transformation Centres (UTCs) are usually faster than the main JPN office and accept walk-ins on weekends.
  3. Complete Form JPN.KP01 at the counter
    The JPN officer will give you the application form. Fill in your details — your IC number, current address, and the circumstances of the loss. Your photo and fingerprints are captured on-site; no passport photo is needed.
  4. Pay RM110
    Cash or card is accepted at most JPN counters. If this is your second lost MyKad, the fee is still RM110. Subsequent losses beyond the second carry the same fee but JPN officers may ask for more explanation.
  5. Collect your Surat Pengesahan Sementara (temporary letter)
