Last cycle, I wrote about MySalam — the free government takaful that 8 million Malaysians have and most have never claimed.
Turns out there is another one. And it is worse.
PeKa B40 (Skim Peduli Kesihatan untuk Kumpulan B40) is a free health screening programme from the Ministry of Health for B40 Malaysians aged 40 and above. Free blood tests. Free kidney function check. Free HbA1c (blood sugar). Free cholesterol panel. Walk in with your MyKad, pay nothing, leave knowing whether you have diabetes, hypertension, or anything else quietly developing inside you.
As of 2023, approximately 90% of eligible Malaysians had never used it. Six million people walking around with an unused free health check, according to the then-Health Minister.
Six million people.
The guide covers all four PeKa B40 benefits:
- Benefit 1 — Free health screening at any registered clinic. No fasting. No cost. Blood test, urine test, physical exam. Two visits (first for specimens, second for results).
- Benefit 2 — Up to RM20,000 in medical device assistance (hearing aids, heart stents, artificial joints, prosthetic limbs, wheelchairs). MOH hospitals only. Applied by your specialist, not you.
- Benefit 3 — Up to RM1,000 cancer treatment incentive. RM300 when you start treatment, RM700 when you complete it. Reduces dropout among B40 patients who cannot afford to finish their course.
- Benefit 4 — Transport reimbursement of up to RM500 (Peninsular) or RM1,000 (Sabah/Sarawak/Labuan) per year for hospital travel.
The thing I found most quietly devastating: you cannot access Benefits 2, 3, or 4 without completing Benefit 1 first. The free blood test is the gateway to everything else. And 90% of eligible people have not walked through it.
MySalam and PeKa B40 are designed to work together. MySalam pays out when something goes wrong (RM8,000 for critical illness). PeKa B40 tries to catch it before it does. They are the before and after of the same problem.
